Social Worker - Perinatal Care Coordination - Days
at Women's Hospital
The Social Worker serves as an integral member of the healthcare team providing assessments, service coordination, information and referral to community resources to meet the complex needs of patients and families in the OB/GYN clinic setting. SBIRT (Screening, Brief Intervention and Referral to Treatment) will be implemented in two additional OB/GYN clinics. The social worker serves as the brief interventionist; responding to all positive screenings evaluating substance use, domestic violence, social determinants of health, and depressive symptoms. The clinic social worker also case finds patients based on social work competencies to asses for financial needs, history/current substance use or mental illness, transportation issues, functional impairments, etc; as well as respond to consults based on risk assessment indicators and/or provider request. In addition, the social worker provides crisis intervention, grief/bereavement counseling, problem solving, stress reduction and patient advocacy
Education: Requires Master's degree from an accredited School of Social Work with LCSW (Licensed Clinical Social Worker) license required
Job Knowledge: Must have considerable knowledge of crisis counseling in a hospital setting, a good knowledge of community resources (including financial assistance programs), and the legal aspects of social service delivery.
